In order to continue to receive your scholarship each year, you must submit an unofficial transcript by July 1st of the year following your scholarship award. Continued eligibility includes evidence that you maintained a 3.00 grade point average on a 4.00 scale. Please upload your unofficial transcript below.
In order to continue eligibility for the scholarship, you must also submit proof of continued enrollment as a full-time student in a two-year associates or four-year undergraduate degree program at an accredited college or university. Please upload your next semester course schedule below.
